OUR SERVICES

Case management provides older adults, families, and caregivers the support and practical resources they need to partner in making decisions. Our services are available for persons over the age of 65 living in Lehigh or Northampton County and their families. Whether it’s dealing with home maintenance, barriers to meal preparation, maximizing benefits, advocacy, support during periods of transitions, social isolation, or connection to information and referrals, JFS Cares About You.

If you or a loved one is struggling with the following quality of life issues, Older Adult Case Management can provide support. We look at the big picture to help clients and their families come up with a personalized care plan.

In-Home Support

We help maximize resources for in-home support so that our clients can continue to safely age in place.

Access to Food

Volunteers and staff help clients facing food insecurity and barriers to meal preparation.

Housing

We help clients who are living independently or transitioning to long-term care residences.

Combating Social Isolation

Staff and volunteers develop strong relationships with clients through our friendly caller and visitor programs and more.

Advocacy and Support

We work on behalf of our older adult clients to make sure needs are known and addressed.

Spiritual Needs

Clients maintain a feeling of connection to the Jewish community through JFS programming, led by volunteers and local clergy to celebrate Shabbat and Jewish holidays.

End-of-Life Needs

Our trained staff helps clients with their end-of-life planning, including advanced directives and Five Wishes.

Family Support

Coping with the challenges of an aging loved one can be difficult. Our staff provides support to families of clients, both locally and out of town through Zoom.

Information and Referrals

JFS maintains strong connections with a wide range of community agencies and refers clients when appropriate.
