Older adult case management can help you navigate the complex maze of aging services. A licensed clinical social worker will be your partner in developing a care plan that meets your specific needs.

Case management provides older adults, families, and caregivers the support and practical resources they need to partner in making decisions. Our services are available for persons over the age of 65 living in Lehigh or Northampton County and their families. Whether it’s dealing with barriers of meal preparation, maximizing benefits, advocacy, support during periods of transitions, social isolation, or connection to information and referrals, JFS Cares About You.

If you or a loved one is experiencing challenges with the following quality of life issues, Older Adult Case Management can provide support. We look at the big picture to help clients and their families come up with a personalized care plan.

In-Home Support

We help maximize resources for in-home support so that our clients can continue to safely age in place.

Access to Food

Volunteers and staff help clients facing food insecurity and barriers to meal preparation.

We help clients who are living independently or transitioning to long-term care residences.

Combating Social Isolation

Staff and volunteers develop strong relationships with clients through our friendly caller and visitor programs and more.

Advocacy and Support

We work on behalf of our older adult clients to make sure needs are known and addressed.

Spiritual Needs

Clients maintain a feeling of connection to the Jewish community through JFS programming, led by volunteers and local clergy to celebrate Shabbat and Jewish holidays.

End-of-Life Needs

Our trained staff helps clients with their end-of-life planning, including advanced directives and Five Wishes.
